Project Name: Detect, Perfect, Protect, Lipid Pathway at Harefield Hospital

Project Summary:

The aim of the project is for the Harefield team to provide enhanced tertiary lipid services.  A multi-disciplinary health professional team will provide tailored care for patients with lipid disorders.

The main objective of the CWP is to:

  1. improve the quality of care for patients at risk of ASCVD with lipid disorders
  2. support the early identification, review and medical optimisation of patients with or at risk of atherosclerotic cardiovascular disease (ASCVD) due to lipid disorder.

Planned Milestones:

  1. Collection of baseline data, in line with the above Project Outcome Measures & Data Collection table
  2. Confirmation of clinical and operational pathway, policy and protocol creation.
  3. Confirmation of recruitment of 2 Nurse Specialist roles
  4. Collection & submission of 3 months clinical activity data. Project Review meeting to discuss project progress.
  5. Collection & submission of 6 months clinical activity data. Project Review meeting to discuss project progress.
  6. Collection & submission of 9 months clinical activity data. Project Review meeting to discuss project progress. Development of business case.
  7. Collection & submission of 12 months clinical activity data. Project Review meeting to discuss project progress
  8. Analysis of CWP data, submission of Final Project Report, Submission of Outcomes Summary

Outcome Summary:

Increased numbers of patients at Harefield Hospital had improved access to lipid management care leading to optimal diagnosis, and management of ASCVD treatments.

Key Project Outcomes Data:

Reduction in Wait time
0-3 months: time from referral received to first appointment for NICE approved injectable treatments, was reduced from 22 to 12 weeks. This was maintained throughout the project until the final 3 months where the waiting time increased to 18 weeks. This was due to the impact of junior doctor strikes as well as the roll out of EPIC (electronic prescribing and medicines administration system) across the trust. 

1602 patients were invited for review.

380 patients accessed injectable therapies (high did not attend (DNA) rates for review appointment, which would have had a knock on effect on numbers of patients accessing treatment). High DNA rates attributed to lack of patient education and awareness about their condition. 

Outcomes:

The introduction of 2.0 FTE band 7 nurses allowed the development of the familial hypercholesterolemia (FH) Cascade testing service, meaning patients from North West London and their families had access to a full FH service closer to home. 

Nurses from the existing service were able to complete Prescribing courses due to the extra capacity created. 

Pressure on the existing team to work extra hours was alleviated somewhat. 

1602 patients were invited for a lipid review in a timely manner. They may otherwise have been kept on a waiting list or referred back into primary care and not had access to optimal care.

Conclusion:

Although this project was considered to be successful in that both patients and the service benefitted, due to financial constraints in the Trust, sustaining the nurse roles in the service has not been deemed possible.
